You may also fill out the forms online prior to printing and then save a copy to your computer so you can print out a new copy any time you need one. You must have the latest version of Acrobat Reader in order to save a copy.

• Permission to Dispense Medication Form (Required each year in order for the school to be able to give your child medication during the school day.)
